The 5 Times Why technique helps teams drill down into the real cause behind an incident, not just the surface-level symptoms. It’s simple, structured, and surprisingly effective.

In Safety Dashboard, we’ve made using the 5 Times Why method easier than ever. When logging an incident, you can immediately launch a 5 Times Why form. Each “why” leads to the next, with branching options to identify which lines of reasoning are relevant—and which ones are dead ends.
